Popliteus muscle strain


Findings

Diffuse discomfort behind the knee, occasionally with tenderness at the insertion on the lateral aspect of the femur, at the palpable notch. The “figure-of-four” sign and resisted internal rotation may be painful.

Cause

Strain of the popliteus muscle, which runs from its tendinous origin at the lateral femoral condyle to its muscular insertion along the back of the tibia. The muscle may be injured at the posterior aspect of the knee and, sometimes, strain the tendon at its insertion. It is usually strained in a twisting movement across the knee and, very rarely, the tendon is avulsed.

Treatment

a)      Ice, for the muscle, is not advised due to the proximity of the popliteal neurovascular vessels, but may help the insertion.
b)      Electrotherapeutic modalities and, rarely, cortisone to the popliteus origin to settle inflammation.
c)      Massage to organize scar tissue at the origin.
d)      Proprioceptive balancing.
e)      Hamstring ladder and knee ladder .

Sports

The injury is usually found in change of direction and contact sports. Many sports people play through games, with discomfort and some limitation to performance, whilst it settles.
